Polycystic ovary syndrome presents a multisystem endocrine disorder requiring integrated assessment across hormonal, metabolic, reproductive, and psychological domains. UK clinicians diagnose it using Rotterdam criteria (two of three: oligo/anovulation, clinical/biochemical hyperandrogenism, polycystic ovarian morphology on ultrasound), after excluding mimics such as thyroid dysfunction or hyperprolactinaemia.
Presentation often occurs in adolescence or early adulthood with irregular menses, hirsutism, acne, or infertility. Historical male-biased research frameworks have contributed to diagnostic delays, as symptoms were sometimes framed through general rather than sex-specific lenses. Lifetime risks include insulin resistance (common), progression to type 2 diabetes, and elevated cardiovascular markers. Pregnancy complications such as gestational diabetes, hypertensive disorders, preterm birth, and caesarean delivery show increased odds.
Management begins with lifestyle optimisation. Pharmacological options include combined oral contraceptives for cycle regulation and androgen suppression, metformin for metabolic features where indicated, and clomifene or letrozole for ovulation induction. Long-term monitoring should address metabolic and cardiovascular risk, with attention to psychological wellbeing given higher reported rates of anxiety and depression.
NICE and international guidelines emphasise holistic, life-course care rather than siloed symptom management. Variation persists in testing sequences, ultrasound reliance, and follow-up, particularly for adolescents where diagnosis requires caution (typically after two years post-menarche).
Women’s Health Hubs and the renewed Women’s Health Strategy seek to improve access and coordination, shifting more care toward primary and community settings while addressing gynaecology waiting times and fragmented pathways.
